In my case it was the cysts I had there. The cysts were caused by the endo.

When my right ovary was removed it was doubled in size due to swelling. I did notice more swelling during my period. So far my left ovary doesn't seem to swell so bad.

Yes definitely, at ovulation and with a period. Even after surgery including ovarian cyst removal. Visible swelling

I was checked several years ago through an ultrasound( way before I was diagnosed with Endo) and they didn't find anything. He didn't check during my surgery, but I wonder if it is kind of like Endo in that it can only be found through surgery instead of an ultrasound. I was diagnosed with PCOS because of my hormones, but with my hormones being crazy due to the PCOS I could have developed one even though the pain and swelling in my ovaries has not changed since I first got my period at 13.
